George - I too also work for a large corporation and we image all of our
documents. One thing to consider in terms of using screenshots as "proof
that the member saw the form" is that a screenshot is just digital data and
is very easily forged. If you need to prove that the member saw this
document for legal reasons, I do not believe a screenshot would suffice in
court.

This may not be the issue in your situation - just thought I would post an
observation.....
