The two providers I use that have been stable and work nicely with
asterisk are Teliax and Voipjet (no did's).
I also use Junction Networks but haven't use them heavily enough to
recommend them.

Supposedly you can plug the phone output from asterisk into an X100P or
tdm card. If you want to be able to receive/send more than one phone
call you'd have to sign up for their softphone option which costs
extra. For my money I'd pick someone else since they don't support
asterisk directly.

    LiveVoip (bankrupt)
    VirtualPhoneline 
    Sixtel/IAX.cc
    Broadvoice
