Pretty print code with function index?

doxygen? I haven't used it to print stuff to paper but it really documents your code to cross-referenced HTML or XML.

a2ps comes awfully close (and I do remember using it back in the day). It

do everything my memory dictates *except* print out the index.

This was back in 1995 or so (give or a take a few years) and I ran it on a

SunOS box. It *may* have been a proprietary app.

Anybody else remember such a gem?
