Cacti with rrdtool works pretty well for me.
Here is an example of my system graphs from cacti. Some SNMP, some script
based.

(the actual cacti interface is way cooler than me just putting the rrd
images up on a wiki)
mrtg is pretty nice too, but the graphs are a little fancier with rrdtool.
