GNU Enterprise does this nicely.

You simply create an .rtf document in editor of your choice using
annotation for the fields you wish to be "merged". You make simple xml
file that defines the mapping from your datasource to your document and
you are good to go.

You can then have output go to printer, email, fax or other device. The
documentation is not super great, but you should readily be able to take
the mail merge sample from gnue-reports and get things working rather
quickly.
