Thanks for the suggestions. Unfortunately (actually fortunately), they showed nothing to be wrong.
This got me thinking. I swapped out the "New" D-Link DFE-503TX+ NIC with a proven LinkSys NIC (borrowed from a LTSP client. boot ROM removed) and it works.
Thanks, your tests got me questions the NIC.
