...and to answer your second question, YES, qwest owns the "local loop"
into your building from the street. Therefore, any service provider
needing access to your building via copper lines, will be relying on
qwest.

Cable companies don't use the qwest copper lines, they use their own
coax and/or fiber plant if it is available in your building.
Occasionally, other service providers will extend their own fiber plant
into buildings that can provide enough revenue to justify it. In this
rare situation, you can also bypass the qwest local loop.

Of course, there are now a few wireless service providers in the area
that can bypass the qwest local loop with no wires at all. ;-)
