that of course ignores interaction with apache mods and other things
such as certificates, authentication methods, referral references,
statistical data etc.

The fact is, that with a fairly loose apache configuration and a proper
pointer in DNS, it can appear that there is no difference between the
www.domain.tld and domain.tld but just the fact that when I typed it in
this email - the www.domain.tld is a clickable link and the domain.tld
is not a clickable link (without the http://) is enough to convince me
that it isn't generally worth doing.
