Inside firewall mail server questions

While I haven't set up all the things you have listed, I did do a basic
(and probably wrong in a couple of areas) presentation on sendmail.
http://luna.flagstaff.az.us/howto/sendmail.html that even if you don't use
sendmail might give you an idea how it can be done.
