Marvin Fretwell and I have been looking into this. I did a fair amount of research today and wrote a rather speculative paper to describe what I found. It is clearly a draft and not ready for prime time. But it is only 2 pages so I will send you a copy off list. Let me know if it is helpful or whatever you might find is wrong in it.
