Mutt has built-in IMAP and POP support, but the POP support is pretty basic,
so most people use fetchmail.

You do need a MTA to send mail, but you don't need to use one of the
complicated ones.  There's a program called "nullmail" that does nothing but
send your mail to a relay.  There's a link to it from the mutt website.
