Adding to the list of possibilities after a quick search:

Request Tracker - http://www.bestpractical.com/rt - Looks well liked and
is the most popular issue tracking solution on Freshmeat.net.  Even has
an ORielly book.

OTRS (Open source Ticket Request System) - http://otrs.org/ - Brief
overview look promising.

CRM-CTT - http://www.crm-ctt.com/ - Looks interesting but have only read
the home page so far.
