need help troubleshooting software raid failures

I would format the /dev/sda over and recreate you raids again.

after that, if it happens again, replace either the controller or /dev/sda
and repeat creating the raids.

My guess is the drive is going (just a guess)

So maybe do some extended testing on the drive to find out how bad it is.
