You may want to ask the Scribus project about this. I recall at one time 
there was discussion on importing PDFs. Scribus can export PDFs that have 
forms support.

Also as of last month there was no work on this for Evince. But I don't 
see any "forms" mentioned in its backend's poppler TODO list and only see 
very little code related to Forms.
