Microsoft USB Optical IMPS/2

You mention having the ZAxisMapping option set, but you didn't mention
anything about telling X how many buttons you have.

XFree86 ver >= 4.0
	Option	"Buttons" "5"
	Option	"ZAxisMapping"	"4 5"
