Try:

ipchains -A ext-in -j ACCEPT -p tcp -s $MAILIP -d $EXTIP smtp
ipchains -A ext-out -j ACCEPT -p tcp -s $EXTIP 25 -d $MAILIP

Try to open the smtp 25 for your mail server only.
