so far, I have been unable to find a ballot template that would allow me to mark the appropriate place on the ballot without assistance.

Actually, they are. 

Every poling place (at least in Maricopa County) has at least one voting machine that has headphones, has braille ballots, large-type, touch-screen voting.....and have had for about 4 years or so, if memory serves......I am not sure about the other Counties - you would have to check with them.  I do, however, think that they are mandated, by law, to provide them for those who need them.
