This is what I get as well, and I'm in a totally different net block.
(see .sig for which one.)  The wire carrier is Qwest, naturally.  I
see nothing useful when looking at the packet dump in wireshark.
traceroute gives me bos-edge-03.inet.qwest.net as the last valid 
hop before * * * city.

You're right, I'm also seeing the Precondition Failed and I'm on XO and some other networks across the country. Looks like either Qwest or Chase is having issues.
