I have a grandstream 2000 that I use with broadvoice.  Actually, it's 
misconfigured somehow so calls will ring or dial out but no one can ever 
hear me.  I suspect I could somehow remedy it, but its funnier broken.

It may not be you :)
Broadvoice is a horrid voip provider.  I used them for about 6 months, it was hit or miss; I was never happy.
Check that you're forwarding the right ports to your phone (or asterisk) and make sure it has a static ip inside your network.

I forward the following ports, you should definitely forward the first 3:
TCP/UDP 5060-5080
rtp 10001-19999
IAX2 4569
FOP 4445
Stun 3478
IAX 5036 (IAX v1)
HUD 6600
