try Xournal* as an alternative PDF tool - Xournal does not fill in the
form, but it will let you accomplish the visual part** which can be
returned as another PDF or printed to paper. ie it adds a graphic on
top of your PDF.

* you can add text anywhere over your original PDF, or, if careful, on
the lines printed for your response - As an added extra, have fun with
the check-boxes as cursor image alignment isn't great.
