I'm on my phone; but from memory....turn off reverse dns lookups in psa_smtpd (think that's it); you add -Rt0 to the rbl line. It seems absolutely pointless to have spamassassin check outbound e-mail. If you want to solve the problem, you are going to have to up the logging level perhaps both with PHP and with qmail to see what the errors how to know how to solve them.
