you should have a file '/etc/modules.conf'.  thats where you put them.   
in terms of names, for the nic cards, they are ethx where x = 0-however 
many cards you have.  you cant really change that name.  for the module 
name, its the 'driver' for you card.  they one you listed in your previous 
email.
