I haven't played around Apache lately but here goes: the action field must be something like: http://hostname.com/var/www/cgi-bin/scriptname.cgi, since the script is called from a website it must be a link to another website, in this case your script. also make sure that your script is in a cgi-bin directory that has the correct permissions.
