Linux and Libre office questions

Here's a PDF version of your form with an embedded Libreoffice draw 
document in it so that is can be edited.  It's a form that can be filled 
out in a PDF reader, or in libreoffice.  It's not exactly like what you 
had, but it should serve as an example of how to make fillable forms.
