Personally, I need more information about what formats you are dealing with.

1. What is the original format of the documentation?

2. What format, if different than plain text, are you trying to get to?

3. What is the overall goal?  A book?  A web page?

I am not an "expert" in this area but I think answers to these questions
might shake someone loose to answer them.
